Qualifications

• Graduate from an ACPE-accredited college of pharmacy or equivalent.
• Two years of ambulatory care clinical experience as part of a multidisciplinary team in the last five years, or one year post-doctoral residency.

Responsibilities

• Provide medication therapy management and ensure safe, effective drug therapy.
• Collaborate with a multi-disciplinary medical team to improve population and patient outcomes.
• Manage MTM-related documentation and billing in ambulatory settings.
