Job summary

HealthPartners is seeking Pharmacists to join its MTM team to improve population-based and patient-specific medication outcomes in ambulatory care. The role will involve comprehensive MTM and collaboration with a multidisciplinary team across Dermatology, Asthma & Pulmonology, Neurology, Infectious Disease, and Oncology.

The position requires Minnesota licensure, ambulatory care experience, and the ability to work a hybrid schedule with in-person and remote clinic days.

Qualifications

• Graduate from an accredited college of pharmacy or ACPE-approved program with didactic and clinical elements.
• Two years of clinical pharmacy experience in ambulatory care as part of a multidisciplinary team within the last five years, or one year of a post-doctoral residency.

Responsibilities

• Provide comprehensive medication management (MTM) to improve patient outcomes.
• Collaborate with a multi-disciplinary medical team in ambulatory care settings.
• Serve as a clinical specialist within designated focus areas (Dermatology, Respiratory, Neurology, Infectious Disease, Oncology).
