Job summary

HealthPartners Park Nicollet Clinic in Burnsville (and HealthPartners Specialty Center 401 in Saint Paul) is seeking a Pharmacist to join our MTM team. This role focuses on improving medication outcomes through collaboration with a multidisciplinary ambulatory care team and providing safe, effective drug therapy.

The position requires a Minnesota Pharmacist license (Wisconsin license within 6 months) and experience in ambulatory care; some remote work days are available and a schedule

Qualifications

• Graduate from an accredited college of pharmacy or ACPE-approved program with didactic and clinical elements.
• Two years of recent clinical ambulatory care experience in a multidisciplinary team, or one year post-doctoral residency.
• Current Minnesota Pharmacist license; Wisconsin license within 6 months of start date.

Responsibilities

• Provide safe and effective drug therapy and MTM services to improve outcomes.
• Collaborate with a multidisciplinary medical team in ambulatory care.
• Contribute to population-based and patient-specific medication optimization.
