Job summary

HealthPartners is hiring Pharmacists to join our MTM team in the Twin Cities metro area. The role focuses on comprehensive medication management in ambulatory care and collaboration with a multidisciplinary medical team.

Ideal candidates have a Pharm.D. and ambulatory care experience; licensure in Minnesota and Wisconsin within 6 months of start. This position offers a hybrid work model with in-person and remote clinic days.

Qualifications

• Graduated from an accredited college of pharmacy (Pharm.D.) on/after May 1996 or ACPE-approved program.
• Two years of ambulatory care clinical pharmacy experience or one year post-doctoral residency.

Responsibilities

• Provide MTM and medication management in ambulatory care.
• Collaborate with a multidisciplinary team to improve patient outcomes.
• Maintain licensure in Minnesota and obtain Wisconsin license within 6 months if hired.
