Qualifications

• Graduate from an accredited school of nursing with current licensure as a Registered Nurse in Minnesota.
• Basic Life Support (BLS) certified.
• Excellent communication across disciplines.

Responsibilities

• Perform nurse visits and support quality initiatives.
• Assist with patient flow and rooming, injections, and patient inquiries.
• Reconcile medications and coordinate with providers and pharmacies.
• Facilitate referrals to internal and external providers.
• Administer vaccines and manage immunization schedules.
• Coordinate care plans with patients and families.
