Ortonville Area Health Services is seeking a compassionate Registered Nurse (RN) to join our clinic care team in Minnesota. The role emphasizes collaboration, autonomy, and a focus on chronic care management to support patients on their health journeys.

You will provide direct patient care across multiple clinic teams, participate in comprehensive patient care, and work with providers, LPNs, CMAs, and RN Care Managers.

Qualifications

• Graduate from an accredited school of nursing.
• Current Registered Nurse (RN) license in Minnesota.
• Clinic experience preferred but not required.
• Strong leadership, communication, planning and problem-solving skills.

Responsabilités

• Provide direct patient care across multiple clinic care teams and adapt to daily staffing needs.
• Participate in and evaluate total patient care through nursing duties and chronic care management.
• Collaborate with providers, LPNs, CMAs, and RN Care Managers to deliver high-quality care.
• Utilize OneChart for documentation and care coordination.
• Assess and interpret symptoms, report patient status, and initiate appropriate interventions.
