Job summary

Logan Health’s Ambulatory Pharmacist will support patients on oral oncology treatments, ensuring continuity of care through monitoring, counseling, and medication reconciliation. Qualifications

• PharmD from an ACPE-accredited college required.
• Current Montana Pharmacist license required.
• Oncology pharmacy specialty certification preferred.
• PGY2 in Ambulatory Care/Oncology preferred.
• Accurate data entry and analytical skills required.

Responsibilities

• Evaluate and verify orders for therapeutic appropriateness and dosing.
• Provide pharmacotherapy recommendations to care teams.
• Monitor lab results and manage adverse effects for oncology patients.
• Perform medication reconciliation and patient counseling.
• Coordinate with specialty pharmacy for medication access and availability.
• Serve as clinical resource to providers and nursing staff.
