Job summary

Fairview Health Services seeks a MTM Float Pharmacist to deliver Medication Therapy Management across multiple clinic locations with both in-person and virtual care. You will evaluate prescribed medications, OTC products, and immunizations, partnering with providers to resolve therapy issues and document in the EMR or MTM software.

Responsibilities include ensuring drugs are indicated, effective, safe, and convenient; taking histories; identifying adverse events; and applying MTM program

Qualifications

• B.S./B.A. in Pharmacy or PharmD from an accredited school of pharmacy.
• PGY1 residency or 3 years of experience as a practicing pharmacist in a clinical setting.
• Working knowledge of third party pay plans and patient drug procurement programs; excellent customer service and communication skills; ability to manage time and people effectively.

Responsibilities

• Provide Medication Therapy Management to patients as outlined in the MTM practice model.
• Ensure all drugs are indicated, effective, safe, and convenient for the patient.
• Obtain histories and apply these along with other patient data in the clinical evaluation.
• Identify, resolve, and prevent medication therapy problems.
• Collaborate with providers under the MTM program to optimize therapy and documentation.
