Job summary

Advocate Health in Winston-Salem seeks a Clinical Pharmacist Float to support our Community Pharmacy network. This full-time position covers multiple community sites with a rotating first shift (7:00 am–7:30 pm) Monday–Friday and weekend/holiday rotation. A sign-on bonus is available for eligible candidates.

You will provide direct patient counseling, oversee medication therapy, and collaborate with the healthcare team to maintain safety and formulary adherence across sites.

Qualifications

• Pharmacist license in practicing state or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Provide counseling to patients and caregivers on medication therapy, adverse effects, compliance, and proper use.
• Design, monitor, and evaluate patient-specific pharmacotherapeutic regimens aligned with literature and policies.
• Review and accurately interpret medication orders and prescriptions.
• Participate in management of medical emergencies when applicable.
• Support medication safety and quality improvement activities and report events via appropriate channels.
