Job summary

Advocate Health in Charlotte, NC, is seeking an Advanced Practice Provider (APN/PA) to join the Department of Surgery in a primarily ambulatory specialty setting. You will deliver autonomous outpatient management for post-operative and surveillance patients, with focus on complex wound and ostomy care, in collaboration with surgeons.

The role emphasizes patient-centered care, continuous education, and opportunities to participate in research, quality improvement, and specialty program

Qualifications

• Board-certified ACNP or PA.
• Surgical experience.
• 1 year of experience preferred.
• Ability to independently manage complex patients.
• Interest in specialty program development.

Responsibilities

• Provide autonomous outpatient management for post-operative patients, surveillance populations, and patients requiring complex wound or ostomy care.
• Conduct histories and physical examinations, perform ongoing clinical assessments, develop and implement treatment plans.
• Provide continued patient education and support.
• Coordinate transitions across care settings and serve as an advocate for patients throughout their care journey.
• Participate in quality improvement initiatives, workflow development, and process improvement efforts.
