Job summary

Yale New Haven Health is seeking an entry-level clinical pharmacist to support the Department of Pharmacy. The role emphasizes patient-centered care, CHAMP behavior, and participation in a High Reliability Organization (HRO) to prioritize patient safety.

The successful candidate will review medication orders, apply pharmacology knowledge, and work with pharmacy automation. Licensure in Connecticut or eligibility is required, with potential ASHP PGY1 residency experience preferred.

Qualifications

Responsibilities

• Provide customer service quality and promote CHAMP behaviors.
• Review medication orders with consideration of patient characteristics including labs and vital signs.
• Demonstrate competency in pharmacy automation and technology as applicable to the work area.
• Adhere to department policies, procedures, protocols, and guidelines to ensure safety and quality.
• Educate and train pharmacy staff, students and residents, and healthcare team members.
• Contribute to departmental cost-reduction or revenue generation.

Responsibilities

  • Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs

Qualifications

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

Experience

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

Licensure

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

Special Skills

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
