Job summary

InnovAge seeks a licensed clinical pharmacist to lead the development of clinical pharmacy services across primary care, focusing on medication management, therapy optimization, and alignment with individual goals of care.

You will collaborate with primary care providers, drive deprescribing, participate in IDT meetings, and use data to improve safety, effectiveness, and cost-efficiency of medication use within CMS and PACE guidelines.

Qualifications

• Pharmacist licensure in state of practice.
• Residency or ambulatory care experience.
• Board certification within 3 years of hire.
• Ambulatory care experience and multidisciplinary team experience preferred.

Responsibilities

• Provide direct patient care focused on medication management, consultation, and education.
• Co-manage medication therapy with primary care teams and optimize regimens.
• Lead initiatives to improve medication use and support cost-effective therapies.
• Promote deprescribing and reduce medication burden for high-risk participants.
• Participate in interdisciplinary team meetings to improve safety and outcomes.
• Use data to identify medication-related risk and guide targeted interventions.
• Serve as center medication expert providing drug information and education.
• Address medication access barriers with operations and vendors.
• Contribute to quality improvement and compliance with CMS, PACE, state, and federal requirements.
• Document all clinical activities and interventions in the medical record.
