Job summary

Live Well Homecare Agency, LLC seeks a Clinical Pharmacist to assess, monitor, and adjust medication therapy for patients based on conditions and age-specific criteria within a collaborative community hospital setting.

You will supervise support personnel, counsel patients on drug therapy, and oversee procurement, storage, and disposal of medications while ensuring accurate preparation and dispensing.

Qualifications

• Bachelor’s Degree in Pharmacy required; Doctorate preferred.
• Minimum of 2 years of experience; supervision experience is not mandatory.
• Licensing as a Pharmacist and Medication Therapy Services certification in MO required.
• Strong knowledge of medication management, clinical assessment, and regulatory compliance.

Responsibilities

• Manage workflow, overseeing technicians, interns, and volunteers to ensure accuracy and regulatory compliance.
• Participate in departmental meetings and serve as a resource for drug information, staff training, and transitions of care.
• Evaluate medication orders for appropriateness and document activities and communications.
• Identify opportunities to improve efficiency and safety within medication use processes.
