Yale NewHaven Health in Connecticut is seeking an entry level clinical pharmacist to support the vision, mission, and goals of the Department of Pharmacy and to demonstrate competency in core clinical pharmacist skills. This role emphasizes CHAMP behaviors, patient safety, and collaboration with the health care team, with CT licensure eligibility.

Qualifications

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
  • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience desirable.
  • Hospital pharmacy experience is preferred.

Responsibilities

  • Demonstrate CHAMP behaviors and promote patient safety.
  • Review medication orders considering patient characteristics including labs and vital signs.
  • Demonstrate competency in pharmacy automation and technology within the work area.
  • Adhere to departmental policies, procedures, protocols, and guidelines.
  • Provide education and training to pharmacy staff, pharmacy students and residents, and health care team members.
  • Contribute to departmental cost-reduction and revenue generation.

Responsibilities

    • 2.1Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs
  • 3.Medication Preparation and Distribution
    • 3.1Demonstrate competency in pharmacy automation and technology as applicable to work area
  • 4.Medication Safety, Regulatory Affairs, and Quality Assurance
    • 4.1Adhere to all departmental policies, procedures, protocols, and guidelines
  • 5.Leadership

Qualifications
EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree

EXPERIENCE

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required. Hospital pharmacy experience is preferred.

LICENSURE

Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut

SPECIAL SKILLS

This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
