Yale New Haven Health is seeking an entry-level clinical pharmacist to support the department of Pharmacy. This role emphasizes patient-centered care, medication counseling, and collaboration with the healthcare team across multiple care sites.
The position requires CT Pharmacist licensure, immunization certification within 6 months, and a foundation in pharmacology. You will participate in medication management, safety initiatives, and ongoing education for staff and students.

This is an entry level clinical pharmacist position. The clinical pharmacist supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills.
EDUCATION
(number of years and type required to perform the position duties): Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree
Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required.
CT Pharmacist Licensure required, active and without restrictions. If required, will be responsible for obtaining out of state licenses to register for non-resident pharmacy licenses. BLS (Basic Life Support) certified or obtained within 6 months of hire. Immunization certified or obtained within 6 months of hire.
This individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. Must have expertise in patient management.
Travel to care sites for meetings may be required.
