Responsibilities

• Manage patient flow, triage and direct patient care.
• Liaison between front desk, nursing staff, physicians and clinic manager.
• Develop patient outcomes and goals with provider collaboration.
• Provide nursing support and coordination to optimize outcomes.
• Educate patients and families about care plans and treatments.
• Document assessments and plans in EMR, track progress.
• Float to different clinics as needed and maintain confidentiality.
