Job summary

Qualifications

Responsibilities

• Manage patient flow, triage and direct patient care in ambulatory clinics.
• Coordinate communication between front desk, nursing, physicians and clinic manager.
• Educate patients and families and promote self-management of disease.
• Utilize EMR to document assessments, plans and progress toward goals.
• Float to different clinics as needed and support clinic sessions.
