Job summary

Trinity Health IHA Medical Group, part of Trinity Health, is seeking an Ambulatory CDI Specialist to join our Revenue Operations team. You will partner with providers, coders, and clinical staff to improve documentation accuracy and support risk adjustment coding initiatives for value-based care programs.

You will review HCC documentation, analyze records for chronic conditions, and develop training materials.

Qualifications

• Associate’s degree in business or health care preferred or equivalent.
• Credentials required: AAPC (CRC), AAPC (CPC), or RHIT.
• 2–4 years’ experience in risk adjustment coding.

Responsibilities

• Review HCC pre-visit documentation and provide coding and documentation recommendations to providers.
• Analyze patient records to identify potential chronic conditions and documentation opportunities.
• Ensure diagnosis codes are accurate, complete, and compliant with current risk adjustment coding guidelines.
• Develop educational materials and provide ongoing training to providers on documentation and coding best practices.
• Serve as a resource for complex coding questions and provide mentorship to new team members.
• Support Clinical Condition Documentation program initiatives, performance monitoring, reporting, and strategic goals related to APMs.
• Lead and coordinate multiple documentation improvement projects while collaborating with internal and external stakeholders.
• Identify and escalation documentation and coding trends, issues, and opportunities for process improvement.
