Job summary

Health Choice Network, Inc. is seeking a Clinical Documentation Improvement (CDI) Specialist to join our team in a regular full-time remote role.

The position focuses on Medicare/Medicaid risk adjustment, HCC coding, and documentation improvement across the organization. You will educate providers and coding staff, develop training materials, and collaborate with health plans to drive quality and value-based care initiatives.

Qualifications

• CRC through AAPC or CDI/CCS through AHIMA certification.
• Minimum of 3 years in Medicare/Medicaid Risk Adjustment (HHS-HCC/CMS-HCC or DxCG).
• Strong ICD-10 coding and clinical documentation review experience.
• In-depth medical terminology, anatomy, physiology, and disease processes knowledge.
• Expertise in HCC coding and risk adjustment programs.

Responsibilities

• Serve as SME on ICD-10-CM coding guidelines and Risk Adjustment methodologies.
• Conduct proactive medical record reviews to evaluate documentation quality and diagnosis coding accuracy.
• Ensure patient conditions are accurately documented to support risk stratification.
• Identify documentation and coding trends impacting patient risk scores and value-based care.
