Job summary

TALENT Software Services is seeking an experienced ambulatory case manager in Cambridge, MA to deliver outpatient case management for HUHS patients. You will conduct patient interviews, monitor care utilization, and develop individualized care plans in collaboration with clinical staff.

Responsibilities include communicating with treating clinicians, providing regular clinical updates, educating medical staff on guidelines, and helping identify and resolve care quality issues.

Qualifications

• Minimum of three years of social work experience in a medical setting.
• Familiar with managed care and case management principles.
• Familiar with care needs of medically complex children, adults, and geriatric populations.
• LICSW or LCSW licensure required; LICSW requires credentialing before start.

Responsibilities

• Provides ambulatory case management services to identified patients through interviews, monitoring, and care plan development.
• Collaborates with clinical staff to deliver a comprehensive outpatient care plan.
• Communicates with treating clinicians and provides updates on care concerns.
• Participates in applying Case Management principles and educates staff on guidelines.
• Identifies quality of care issues and assists in developing resolutions.
• Assists in developing reports and resources for other departments.
• Manages a small team handling 40–50 cases, hospitalizations, and advance directive outreach.
