Qualifications

• RN license active in Massachusetts.
• 3+ years of hospital, home health, or ambulatory care experience.
• Knowledge of discharge planning, utilization/quality management, home care or hospice nursing.
• Strong communication and interpersonal skills.
• Proficient in multiple software and EHR systems.

Responsibilities

• Manage PHO Members to reduce acute care episodes and optimize ambulatory care.
• Coordinate and lead interdisciplinary teams to review referrals and create individualized care plans.
• Assess care needs using guidelines and established standards; document and monitor progress.
• Communicate with members, families, physicians, and other providers to ensure safe, patient-centered care.
• Collaborate with PCPs, specialists, and community resources to support coordination and transitions of care.
