Jefferson Health is seeking a professional nurse to join the Primary Care team in Bala Cynwyd, PA. The role focuses on patient-centered care, coordinating with providers and support staff to deliver evidence-based nursing interventions across in-person and electronic communications.

The successful candidate will hold an active RN license in the state and preferably a BSN, with 2+ years in primary care/ambulatory settings.

Qualificações

• Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
• Two-plus years of experience as a nurse, in primary care or ambulatory setting preferred.

Responsabilidades

• Collaborates with primary care providers and support staff to deliver clinical nursing interventions.
• Communicates effectively with patients to address individualized needs and cultural beliefs.
• Performs nursing care and documentation in EMR according to standards and regulations.
• Assists with triage, vaccine verification, and care coordination for abnormal results.
• Participates in team huddles and integrates feedback to improve practice quality and safety.
