Greater Baden Medical Services, Inc. is seeking an Ambulatory Care RN to lead Population Health/Chronic Care initiatives at our Brandywine, MD site. The role emphasizes care coordination, chronic disease management, and preventing readmissions within an FQHC setting.

Responsibilities include patient education, care plan maintenance, PCMH participation, triage, immunization oversight, and collaboration with providers to close care gaps and improve outcomes.

Qualifications

• Current Maryland RN license.
• Bachelor’s degree in Nursing or an Associate’s Degree in Nursing.
• Care management experience preferred.
• Current CPR certification.
• Bilingual is a plus.
• Pediatric experience is a plus.

Responsibilities

• Identify at-risk patients and support self-management of disease and behavior modification.
• Coordinate care plans to meet PCMH requirements and advance population health.
• Educate patients on Diabetes Prevention and Diabetes Self-Management and related topics.
• Monitor ER visits and educate patients on appropriate use of medical services.
• Provide triage, direct nursing care, and support immunization and lab oversight.
