Qualifications

• Bachelor’s degree in BSN or public health/health administration required.
• 3+ years of population health, care coordination, public health, or outpatient settings.
• 1+ years of supervisory or team lead experience in clinical or community outreach.
• Active RN license required if BSN.

Responsibilities

• Direct supervision and mentoring of the Population Health Team.
• Leverage CRISP data to drive care coordination and quality objectives.
• Provide staff training on member health education and health literacy.
• Oversee HEDIS, Value-Based Care, gaps in care and No-Encounter reporting.
• Participate in PCMH frameworks and quality improvement initiatives.
• Monitor and administer member incentive programs for preventive visits.
• Manage disease management workflows and high-risk care coordination.
• Develop health education and promotion programs for chronic conditions.
• Coordinate transitions of care with clinics and hospitals.
• Liaise with Managed Care Organizations (MCOs) to align initiatives.
• Lead community outreach and health literacy events with partners.
• Assess social determinants of health to reduce disparities.
