Job summary

Baltimore Medical System (BMS), Baltimore, MD, is seeking a Registered Nurse Ambulatory Care to support care management and coordinate care for a diverse patient population. You will triage acute needs, educate patients, administer medications, and collaborate across clinical teams in a fast-paced ambulatory setting.

Requires RN licensure in Maryland (or Compact State), and a 4-year degree in nursing; BSN preferred.

Qualifications

• Graduate of an accredited School of Nursing.
• BSN preferred.
• BLS certification required or equivalent.

Responsibilities

• Provide care management and coordination for a patient panel.
• Triages acute needs and provides chronic/acute care education.
• Administer medications as prescribed and support treatment plans.
• Collaborate with clinicians and other clinical team members to optimize patient care.
