Baltimore Medical System (BMS) is seeking a Registered Nurse Ambulatory Care to support care management across ambulatory clinics in the Greater Baltimore area. Working under the Center Clinical Manager, you will provide nursing interventions and execute treatment plans to coordinate patient care, triage needs, and work with clinicians.

Required: graduation from an accredited School of Nursing, Maryland or Compact RN license, BLS, and at least three years of acute/ambulatory nursing experience.

Qualifications

• Graduate of an accredited School of Nursing.

Responsabilités

• Triages acute needs and provides chronic and acute care education.
• Administers medications and addresses patient and provider taskings.
• Care management and coordination of a patient panel under the Center Clinical Manager.
• Collaborates with other clinical team members to optimize patient care.
