Qualifications

• High School diploma, GED, or equivalent.
• Previous experience with appointment scheduling within a medical office setting.
• Familiarity with basic medical terminology.

Responsabilités

• Scrub and manage Medicaid/Medicare MCO rosters to contact disengaged patients.
• Conduct multi-channel outreach to re-engage patients with primary care services.
• Identify barriers to care during outreach and guide patients to scheduling and resources.
• Provide telephonic navigation to reconnect patients with internal financial counselors or eligibility specialists.
• Coordinate targeted outreach campaigns aligned with quality improvement goals (HEDIS measures).
• Navigate the EHR in real-time to secure, reschedule, or change appointments; perform warm transfers when needed.
• Provide comprehensive pre-visit instructions and collect necessary documentation.
• Follow up on hospital discharge and ED visits with nursing and care teams to arrange timely follow-ups.
• Document patient interactions and outcomes in the EMR; maintain data integrity.
• Coordinate hand-offs to Population Health Nurses or on-site CHWs for complex cases.
• Participate in community outreach events to promote primary care services.
