Job summary

Community Health Connections, Inc. is seeking an outpatient Primary Care Nurse in Massachusetts.

The role provides patient-centered nursing care in a busy primary care setting, supporting health promotion, disease prevention, and care coordination across the lifespan. You will work with providers and the team to ensure safe, high-quality care, perform intake, assist with exams, administer meds, manage chronic conditions, document in the EHR, and participate in quality initiatives.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with MA licensure.
• BLS Health Care Provider certification required.
• Ambulatory/outpatient nursing experience preferred.

Responsibilities

• Perform patient intake: vital signs, health history, medication reconciliation.
• Assist providers during examinations and procedures.
• Administer medications and immunizations per orders and guidelines.
• Coordinate care, referrals, and follow-up; provide patient education.
