Community Health Connections is seeking a Primary Care Nurse in a Massachusetts outpatient clinic. The RN/LPN provides patient-centered care across the lifespan, supports health promotion, disease prevention, chronic disease management, and care coordination with the interdisciplinary team.

The role emphasizes accurate documentation in the EHR, adherence to safety and quality standards, and collaboration with physicians, nurses, medical assistants, and staff. Evening shifts are required.

Qualifications

• Registered Nurse or Licensed Practical Nurse license.
• Current MA RN or LPN license.
• Ambulatory medical office experience preferred.
• Bilingual in Spanish and/or Portuguese is a plus.
• Must be available to work at least one evening per week.
• Fast-paced medical office experience desired.
• English communication proficiency required.

Responsabilités

• Take patient intake with vital signs and health histories.
• Assist providers during examinations and procedures.
• Administer medications and immunizations per guidelines.
• Perform wound care, injections, and specimen collection.
• Document in the EHR and ensure quality measures.
