Job summary

Community Health Connections, Inc. in Gardner, MA, is seeking a Primary Care Nurse to provide patient-centered, outpatient nursing care across the lifespan, supporting promotion, prevention, chronic disease management, and care coordination.

You will perform intake, assist with examinations, administer medications and immunizations, conduct wound care, and collaborate with the care team to ensure safe, high-quality care in a fast-paced clinic.

Qualifications

• RN or LPN license in MA (required).
• 1 year ambulatory medical office experience preferred.
• Bilingual in Spanish and/or Portuguese is a plus.

Responsibilities

• Take patient intake: vitals, history, medication reconciliation.
• Assist providers during exams and procedures.
• Administer medications and immunizations.
• Perform wound care, injections, point-of-care testing, specimen collection.
• Monitor patients for acute and chronic conditions.
