Community Health Center, Inc. is seeking a Primary Care Nurse in Hartford to deliver high-quality nursing care in an ambulatory setting. You will work with a clinical team, perform nurse-led visits, and support transitions between care levels while using EHR for timely documentation.

The role emphasizes preventive and chronic care, patient education, and coordination with PCPs. Strong nursing license and willingness to collaborate are required, with full-time hours and site-based duties.

Formación

• Registered nurse, in good standing.
• BSN or ASN required.
• Bilingual preferred.
• Electronic health record experience preferred.

Responsabilidades

• Support primary care of patients under planned care model, including pre-visit planning and nurse-led visits.
• Collaborate with PCP to identify preventive and chronic disease care needs.
• Provide health education during visits and by telephone.
• Assist with exams, treatments, and procedures as needed.
• Document care and coordinate referrals for specialty care as ordered.
• Maintain infection control standards and safe medication administration.
• Monitor patient responses to medications and counsel on self-management goals.
• Operate within CHCI protocols for documentation and patient confidentiality.
