Zusammenfassung

Boston Health Care for the Homeless Program (BHCHP) seeks a professional nurse to provide compassionate, ambulatory care across a range of health conditions. You will build trust with patients, offering education, support, and care focused on medication management, chronic disease management, and preventive health.

The role requires flexibility to travel between BHCHP sites in the Boston area; part-time schedule (20–24 hours) with days/evenings/weekends as needed.

Qualifikationen

• Massachusetts RN license required.
• Minimum of 2 years nursing experience preferred.
• Experience with underserved populations and active substance use preferred.
• Multilingual proficiency preferred – Spanish, Haitian Creole, and Portuguese-Brazil.
• Epic EMR experience preferred.
• Strong medical assessment skills.
• Experience in public health, primary and preventive nursing/medicine.
• Understanding of substance use, trauma, and behavioral health issues.

Aufgaben

• Provide clinical nursing care addressing patients' health needs per BHCHP policies and DPH regulations; includes immunizations, injections, point-of-care testing, and health screenings.
• Collaborate with BHCHP clinical team to address patient health concerns.
• Assess and document ongoing clinical nursing care promptly; report to appropriate personnel.
• Assist patients with medication management and disease understanding.
• Respond to medical emergencies; practice trauma-informed care and harm reduction.
