Responsibilities

• Integrates evidence-based practice, clinical expertise, and patient/family preferences for optimal care.
• Collaborates with clinic care providers ensuring timely communication between primary care and specialists.
• Supports implementation of clinical care pathways and participates in quality improvement.
• Documents all patient interactions and nursing care in EMR per organizational standards.
• Coordinates care for abnormal results, vaccinations, and care management activities.
• Participates in team huddles and applies feedback to practice.
