Job summary

Jefferson Health Northeast seeks an experienced Registered Nurse for ambulatory care within the Primary Care team in Philadelphia. You will use the nursing process to assess, plan, implement, and evaluate care for diverse patients and families.

You will collaborate with providers, support staff, and specialists to coordinate care, implement care pathways, and participate in quality improvement. Strong communication and cultural sensitivity are essential.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred within 3 years of hire.
• Current RN licensure in state of practice; BLS required as per policy.
• 1+ years’ ambulatory nursing experience preferred.
• Commitment to evidence-based practice and quality improvement.

Responsibilities

• Applies the nursing process to provide patient- and family-centered care across physical, psychological, emotional, and social needs.
• Collaborates with primary care providers and supports communication between primary care teams and specialists.
• Helps implement clinical care pathways for common primary care conditions and participates in quality improvement initiatives.
• Delivers patient education on preventive care, chronic disease management, and medication management.
• Documents all patient interactions and care in the EMR per organizational standards.
• Performs care coordination for abnormal results, triage, and vaccination management.
• Participates in team huddles and actively seeks professional development opportunities.
• Demonstrates problem solving and conflict resolution within an inter-professional team.
