• Assess and plan for patient care as part of multidisciplinary team.
• Lead nursing visits and provide education on disease management.
• Identify high-risk patients and coordinate outreach and follow-up.
• Perform medication reconciliation and adherence counseling.
• Triage patients during walk-in or same-day clinic visits.
