Job summary

Bergen New Bridge Medical Center is hiring a Health Population Navigator to support patients through care coordination and social determinants of health. You will connect patients with community resources, coordinate appointments, and document encounters while maintaining strict confidentiality.

The role requires experience in community health or care coordination, a background in behavioral health, and strong communication skills.

Qualifications

• Experience serving complex or vulnerable populations.
• Knowledge of social determinants of health and non-clinical drivers.
• Experience with community-based referrals and closed-loop referrals.

Responsibilities

• Coordinate care with patients and care team members.
• Guide patients to appointments and accompany them when needed.
• Act as liaison with medical providers to ensure adherence to care plans.
• Track attendance, outreach, and follow-up on missed appointments.
• Maintain program database of partners and referrals.
• Maintain confidentiality per policies and comply with reporting requirements.
