Job summary

Bronson Healthcare is seeking an Ambulatory Support Assistant to join the Kalamazoo ambulatory care management team. You will support care coordination, schedule appointments, manage referrals, and connect patients with resources to optimize health outcomes.

Requirements include a high school diploma or GED and at least 2 years in healthcare or human services; familiarity with Epic is preferred. Strong customer service and organizational skills are essential in a dynamic, patient-centered

Qualifications

High school diploma or GED required; healthcare or human services background preferred.
Basic medical terminology knowledge is required.
Experience as referral coordinator, case worker, or similar role is preferred.
Experience in healthcare with adult/pediatric patients in ambulatory/primary care settings desirable.

Responsibilities

Care coordination across the care team and with patients to connect to resources.
Coordinate referrals and follow up on referral status.
Assist in completing forms and applications for patients (e.g., medication assistance).
Ensure insurance coverage for services and communicate with insurers as needed.
Schedule medical appointments for hospital follow-up and care team members.
Maintain documentation for patient encounter activities.
