Job summary

UC Davis Health is seeking an experienced Registered Nurse Case Manager to coordinate care across inpatient/outpatient settings. The role combines telephonic and on-site assessments to manage high-risk patients, with a focus on comprehensive care planning and coordination.

The position supports ambulatory case management and requires strong clinical judgment, chart analysis, and collaboration with diverse teams.

Qualifications

• Two years' RN experience with case management of chronic/complex conditions.
• Two years RN clinical hospital-based experience, inpatient or outpatient.
• California RN license without restriction.
• Graduate of an accredited RN program.
• Experience identifying and analyzing care/service needs for patients.
• Knowledge of nursing care plans and evidence-based guidelines.
• Ability to perform in-depth chart analysis for concise information.
• Ability to conduct telephonic and/or on-site assessments and complex case management.
