Valley Medical Center is seeking a Clinical Pharmacist, Outpatient - Per Diem to support ambulatory care across primary, ambulatory and specialty clinics in Renton, WA. The role emphasizes medication therapy management, patient education and collaboration with a multidisciplinary team.

The pharmacist will participate in transitions of care, ensure alignment with clinical practice guidelines, and contribute to staff development within Pharmacy Services.

Qualificações

• Graduate of an accredited College of Pharmacy; PharmD degree strongly preferred.
• Licensed as a Registered Pharmacist in the State of Washington with license in good standing, or eligible for licensure.
• Completion of an ASHP accredited PGY1 residency required or equivalent practice experience; PGY2 preferred for Oncology.
• BLS for Healthcare Providers active or required within 30 days of employment.

Responsabilidades

• Provide pharmaceutical care to ambulatory clinic patients, including medication therapy management and drug therapy consultation.
• Engage in transitions of care and patient education to ensure safe and effective therapy.
• Collaborate with providers to individualize therapies based on patient parameters and literature.
• Supervise pharmacy residents, technicians, interns and students as applicable.
• Participate in quality assurance and adherence to formulary and clinical guidelines.
