Job summary

Summit Pacific Medical Center is seeking an Ambulatory Clinical Pharmacist to collaborate with the primary care team in a role focused on medication optimization and safety. You will conduct ambulatory visits, provide dosing support for the Anticoagulation Clinic, and contribute to chronic disease management.

The ideal candidate holds a PharmD, with residency or ambulatory experience, and an active WA license; Board-Certified ambulatory care is preferred.

Qualifications

• PharmD from an accredited school of pharmacy.
• Post-graduate year residency or two years ambulatory clinical pharmacy experience.
• Active Washington State pharmacist license.
• Board-Certified ambulatory care preferred.

Responsibilities

• Ambulatory pharmacist patient visits.
• Medication consults with primary care providers.
• Participation in chronic disease treatment protocol development.
• Providing medication information and education to the primary care team.
• Dosing support for the Anticoagulation Clinic.
• Cross-training with the Pharmacy Department.
• Special projects and other related duties as assigned.
