Responsibilities

• Ambulatory pharmacist patient visits.
• Medication consults with primary care providers.
• Participation in the development of chronic disease treatment protocols.
• Providing medication information and education to primary care team.
• Provide medication dosing support for SPMC Anticoagulation Clinic.
• Cross‑training with SPMC Pharmacy Department.
• Special projects and other related duties as assigned.
