Job summary

Lodi Creek Post-Acute in Lodi, CA is seeking a Licensed Vocational Nurse (LVN) to join our dedicated healthcare team and provide compassionate patient care. The role requires a California LVN license and at least 6 months of SNF/LTC experience.

You will work under the supervision of RNs and physicians to deliver structured care plans and ensure patient safety. Responsibilities include administering medications, monitoring vital signs, assisting in exams, and educating patients and families on

SPECIFIC RESPONSIBILITIES, NOT LIMITED TO:

• Provide direct patient care under the supervision of registered nurses or physicians.
• Administer medications, injections, and treatments as prescribed by healthcare professionals.
• Monitor and record vital signs, symptoms, and medical histories of patients accurately.
• Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans.
• Assist physicians and registered nurses during examinations, procedures, and treatments.
• Perform routine procedures such as wound dressing changes, catheterizations, and tracheostomy care.
• Monitor and document patients' progress, reactions to medications, and overall conditions.
• Educate patients and their families on disease management, medication regimens, and post-treatment care.
• Maintain a clean, safe, and organized work environment.
• Adhere to infection control protocols and maintain a high standard of hygiene.

QUALIFICATIONS/SPECIFIC REQUIREMENTS:

• EDUCATION: Graduate of an accredited school of nursing
• EXPERIENCE:
• Must have at least 6 months of experience as an LVN in SNF/LTC.
• LICENSE/CERTIFICATIONS: A current California license as a Licensed Vocational Nurse (LVN) license is REQUIRED
