Lodi Creek Post-Acute in Lodi, CA is seeking a Licensed Vocational Nurse (LVN) to join our dedicated healthcare team and provide compassionate patient care. The role requires a California LVN license and at least 6 months of SNF/LTC experience.

• California LVN license required; active.
• Minimum 6 months LVN experience in SNF/LTC.

Responsibilities

• Provide direct patient care under supervision of RNs or physicians.
• Administer medications, injections, and treatments as prescribed.
• Monitor and record vital signs and medical histories.
• Collaborate with interdisciplinary team to develop care plans.
• Assist physicians and RNs during examinations and procedures.
• Perform wound dressing changes, catheterizations, and tracheostomy care.
• Monitor patient progress and educate patients and families on care.
