Canyon Creek Post-Acute, a 5-star skilled nursing facility in Castro Valley, CA, is seeking a Licensed Vocational Nurse (LVN) to join the care team. You will work under the supervision of an RN or Nurse Manager to deliver high-quality nursing care and contribute to individualized care plans.

Responsibilities include patient assessments, vital sign monitoring, medication administration, wound care, and care coordination with CNAs and other staff.

Qualifications

• Must be a Licensed Vocational Nurse (LVN) in California.
• Six months of experience, ideally in a skilled nursing environment.
• Active BLS/CPR

Responsibilities

• Perform initial and ongoing assessments of residents' health status.
• Monitor vital signs, administer medications, and observe changes in condition.
• Notify RN or healthcare provider of significant changes or concerns.
• Administer medications as prescribed and ensure correct routes and timing.
• Record residents’ responses to medications and document changes.
• Provide basic wound care and other orders from the provider.
• Coordinate with RNs, CNAs, and other staff to plan and deliver care.
• Participate in care planning meetings and contribute to resident care plans.

Position Overview:

The Licensed Vocational Nurse (LVN) in a Skilled Nursing Facility is responsible for delivering high-quality nursing care to residents under the supervision of a Registered Nurse (RN) or Nurse Manager. The LVN works collaboratively with the healthcare team to ensure the well-being and comfort of residents.

• Conducts initial and ongoing assessments of residents' health status.
• Monitors vital signs, administers medications, and observes any changes in condition.
• Notifies the RN or healthcare provider of any significant changes or concerns.
• Administers medications as prescribed by the healthcare provider.
• Ensures accurate dosage, proper route, and timely administration.
• Monitors and records residents' responses to medications.
• Provides basic wound care and other treatments as ordered by the healthcare provider.
• Monitors the healing progress of wounds and reports any complications.
• Works closely with Registered Nurses, Nurse Managers, Certified Nursing Assistants (CNAs), and other healthcare professionals to coordinate care.
• Participates in care planning meetings and contributes to the development of resident care plans.

Qualifications:

• Must be a Licensed Vocational Nurse (LVN) in California.
• Six months of experience, ideally in a skilled nursing environment, is desired.
• Active BLS/CPR
