Job summary

UnitedHealthcare is hiring a Field Care Coordinator (Case Manager) to manage a caseload with low-to-medium complexity medical and behavioral needs. This role emphasizes field visits, home or facility assessments, and collaboration with care teams to improve outcomes.

The position involves travel about 50% of the time within Navajo and Apache Counties, including Show Low, St. Johns, Springerville, and Eagar. A valid driver’s license and MS Office proficiency are required.

Qualifications

• 2+ years of case management experience for members with SMI.
• 1+ years of case management for elderly and/or disabilities.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Driver's License and reliable transportation.

Responsibilities

• Engage members face-to-face or telephonically for comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Provide education and coaching to support self-management and healthy lifestyle changes.
• Support discharge planning and coordinate care transitions after ER, inpatient, or SNF stays.
• Advocate for members to ensure their needs and choices are represented by the care team.
