Job summary

UnitedHealthcare is seeking a Field Care Coordinator to manage a caseload of members with low-to-medium complexity medical and behavioral needs, including those eligible for ALTCS or Long Term Care Services. The role emphasizes field visits and coordination with care teams to promote appropriate service use and better outcomes.

You will travel about 50% of the time across Navajo and Apache Counties, including Show Low, St. Johns, Springerville and Eagar.

Qualifications

• 2+ years of case management experience serving members with Serious Mental Illness (SMI)
• 1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Driver's License and ability to travel ~50% of the time

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs
• Partner with internal care team, providers, and community resources to implement care plans
• Provide education and coaching to support member self-management
• Support discharge planning and Care Transition after ER, inpatient, or SNF
• Advocate for members and families to ensure needs are represented
