Job summary

UnitedHealthcare is seeking a Field Care Coordinator to support a caseload of members with low-to-medium complexity medical and behavioral needs, including those eligible for ALTCS or Long Term Care Services. The role emphasizes field visits, care planning, and collaboration with clinicians to improve outcomes in Navajo and Apache counties, AZ.

You will assess needs, build care plans, educate members, and coordinate transitions while maintaining a patient, adaptable approach in a dynamic field

Qualifications

• 2+ years of case management experience serving members with serious mental illness (SMI).
• 1+ years of case management experience with elderly and/or disabled individuals.
• Proficient in MS Office (Word, Excel, Outlook).
• Access to reliable transportation and ability to travel 50% of the time.

Responsibilities

• Engage members face-to-face and/or telephonically for comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with internal care team, providers, and community resources to execute the care plan.
• Educate and coach members to support self-management and healthy lifestyle changes.
• Coordinate discharge planning and Care Transitions after ER visits or SNF stays.
