Arista Home Care is seeking an ALF HHA Coordinator to serve as the onsite liaison at the assigned assisted living facility. You will manage intake paperwork, onboard new HHAs, and coordinate day-to-day cluster care schedules and coverage to support the plan of care and timely service delivery.

You will maintain accurate records, communicate changes promptly, and collaborate with facility staff and the clinical team to ensure continuity of care.

Qualifications

• High school diploma or equivalent required.
• Active HHA certificate required; must have held active HHA certificate for at least one year.
• Experience in home care, assisted living, caregiver scheduling, or cluster care coordination preferred.
• Experience with patient intake paperwork, caregiver onboarding, EVV, or home care management systems preferred.
• Ability to work onsite at the assigned ALF and manage multiple priorities.
• Strong organizational, problem-solving, customer service, and communication skills.
• Ability to work collaboratively with patients, families, HHAs, facility staff, clinical staff, and leadership.
• Proficiency in Microsoft Office and ability to learn scheduling and documentation systems.
• Ability to maintain confidentiality and follow agency procedures.

Responsabilités

• Serve as onsite liaison between Arista Home Care and the ALF, communicating patient service needs and staffing updates.
• Complete and coordinate required intake paperwork for new patients and obtain signatures.
• Track missing intake documents and escalate delays.
• Coordinate new patient starts with Intake and clinical team for approvals and HHA coverage.
• Orient new HHAs to facility procedures, schedules, and documentation expectations.
• Create and maintain cluster care schedules based on planned services.
• Respond promptly to call-outs and gaps; arrange replacement coverage as needed.
• Maintain accurate patient and HHA rosters and communicate changes to departments.
• Communicate patient condition changes to clinical team and management.
• Coordinate with nursing and Compliance for assessments and supervision scheduling.
• Maintain timely records in agency systems for back-office monitoring.
