Monroe Health & Rehab Center in Huntersville, NC is seeking a Special Care Coordinator to meet memory care residents’ needs and develop engaging activities in the special care unit. You will work closely with operations and clinical teams to maintain high-quality, compliant care.

The role requires two years in long-term care with cognitive decline and active licenses as Medical Assistant, Pharmacy Technician, or Medication Aide, with dementia certification preferred.

Qualifikationen

• Two years' experience in long-term care and with adults with cognitive decline.
• Two years' experience as a Medical Assistant, Pharmacy Technician, or Medication Aide.
• Current unencumbered license in MA, Pharmacy Tech, or Medication Aide in-state.

Position Summary:

The Special Care Coordinatoris responsible to meet memory care residents’ needs and develop and manage activities in the special care unit, in accordance with resident need, current federal, state, & local regulation, health, sanitary & infection control policy, and all company policies and procedures. The SCC works directly with the operation and clinical management staff to ensure the highest degree of care is maintained for each resident at all times.

Education/Qualifications:

• Must possess a current, unencumbered license to practice as a Medical Assistant, Pharmacy Technician, or Medication Aide in the state in which you work.

• Minimum of two (2) years’ experience in long-term care and with adults with cognitive decline.

• Minimum two (2) years’ experience as a Medical Assistant, Pharmacy Technician, or Medication Aide.

• Certified Dementia Practitioner designation and/or Activity Director certification preferred.

Shift & Wage:

• Full-Time Opportunity Available

• Shifts Needed: 1st Shift and On Call
