Job summary

Monroe Health & Rehab Center in Cambria Township, PA, is seeking a Certified Occupational Therapy Assistant to coordinate dementia programming within our interdisciplinary team. You will design, implement, and oversee activities that support cognitive, emotional, and physical well‑being for residents.

This full‑time position requires at least 2 years in long‑term care or memory care and a Dementia Practitioner certification or the ability to obtain it within six months.

Qualifications

• Preferred Certified Occupational Therapy Assistant (COTA).
• Dementia Practitioner certification preferred or obtainable within 6 months.
• Minimum 2 years in long-term care or memory care programming as a COTA or related role.

Responsibilities

Position Summary:

The primary purpose of your job is to coordinate with the interdisciplinary healthcare team to provide comprehensive care and support for residents with dementia and/or dementia-related conditions. You will plan, organize, implement, and oversee specialized dementia programming that aligns with best practices in cognitive wellness. This position supports the overall goals of enhancing the emotional, cognitive, physical, and social well‑being of each resident through structured, individualized, and compassionate programming.

Education/Qualifications:

• Certified Occupational Therapy Assistant (COTA) preferred. May consider Associate or Bachelor’s degree in Human Services, Nursing, Occupational Therapy, Recreational Therapy, or a related field.
• Current certification as a Dementia Practitioner preferred or ability to obtain within six (6) months of hire.
• Minimum of two (2) years of experience in long-term care or memory care programming as a Certified Occupational Therapy Assistant (COTA) or other discipline as noted above.
